Ultrasonic Liposuction/ Lipolysis Plastic Surgery

Liposuction, also known as Lipoplasty, Suction Assisted Lipectomy (SAL), Liposculpture, Lipolysis, or key hole liposuction, is a minor surgical procedure that removes fatty deposits from various areas of the body. It is an effective procedure to remove fat from areas from where it is difficult to shift fat through exercise or diet. Other common areas that liposuction is performed on are:

  • Inner and outer thighs
  • Stomach
  • Buttocks
  • Around Knees
  • Hips
  • Arms
  • Upper and Lower back
  • Double chin and face
  • Waistline
  • Breasts (both male and female)

Mega liposuction is a term used for large volume liposuctions. At a time we can safely remove 10- 15% of body fat amounting to 12- 18 litres of fat and around 15- 20 inches in one session. Dr. Viral Desai has performed a large number of mega liposuctions and has presented many papers on the topic.

There are two types of techniques used for Liposuction surgery. The first is the “tumescent technique” or the “super wet technique”, in which the surgeon injects large quantities of fluid through small incisions. The fluid reduces the risk of bleeding. The fat cells swell with fluid going into the fat layers. A cannula is then passed through the swollen fat cells. A suction through the cannula removes the fatty tissue and the fluid that has been injected. The fluid that is not removed at the time of the procedure will drain post-operatively or remove itself from the body through the patient’s urine. Bruising, swelling and minor discomfort occurs post-operatively; however, this subsides in time.

The second technique is Ultrasound-assisted lipoplasty (UAL) or VASER. With this technique, the surgeon uses a cannula that produces ultrasonic energy. The vibrations from the ultrasonic energy attack the fat cells, thus breaking down the fatty tissue into a liquid form. The liquid is then removed by suction. The very small incisions made to insert the cannula are barely visible.

Dr. Viral Desai generally performs liposuction with the super-wet technique (SWT). Studies show that the SWT with injection of anesthetic and saline solution to the same or double the volume as the estimated fat removal, reduces blood loss and bruising. Sedation and monitoring by an anaesthesiologist or anaesthetist ensures your comfort and safety.

Power Assisted Liposuction (PAL) involves use of power tools to break down and remove the fat rapidly. The recovery is very fast and there in no injury to any other cells.  PAL can be combined with 3 Dimensional Superficial Liposculpture (3D-SLS) and External Ultrasound Assist (XUAL) to give the benefits of all three methods. This leads to a more thorough removal of fat in one stage with contouring as well as decreased pain and swelling. The Power Assisted Lipoplasty Device is the shape of things to come in Lipoplasty Surgery.

Liposuction is helpful when you have areas that fail to improve or excess fat. These are most frequently the outer thighs and hips for women and love handles and the tummy for men.You may be a good candidate for liposuction if you have localized excess fat or desire contouring in any of the following areas:

  • Cheeks, jowls and neck
  • Upper arms
  • Breast or chest areas back
  • Abdomen and waist
  • Hips and buttocks
  • Inner and outer thigh
  • Inner knee
  • Calves and ankles

Age is not the limiting factor for liposuction provided you are in good health. If your skin is stretched and loose from pregnancy or weight loss, then liposuction may not help. In these situations, it is more effective to remove the extra skin, itself. This is especially true in the arms, tummy, inner thighs and buttocks. Dr. Viral can explain the best procedure to improve your appearance at your consultation.

We give utmost importance to patient confidentiality and take strictest care to ensure your privacy. The staff at CPLSS Mumbai understands that it can be difficult for a patient to make enquiries about such a delicate matter. It is their aim to make you feel as relaxed, comfortable and fully informed about the procedure as possible. Our staff will be happy to help you make an appointment with Dr. Viral Desai.Dr. Viral Desai will discuss your expectations from liposuction and will advise you as to whether your expectations are realistic.During the consultation, there are several points that Dr. Viral Desai will need to know about you, including:

  • If you have had any previous plastic surgery or Liposuction
  • Your complete medical history and any medications that you are taking (if you suffer from diabetes, heart disease or high blood pressure)
  • Your current weight and your intentions to lose any weight in the future
  • Your skin texture and elasticity

You should expect to be in your consultation with Dr. Viral Desai for around 30 minutes.

  • You should stop smoking (including the use of nicotine patches), intake of any blood thinning tablets (i.e. aspirin/disprin) and intake of vitamins at least one week prior to surgery
  • Fasting is required for 7 hours prior to surgery
  • You should inform Dr. Viral Desai of any allergic reactions to medications, general anaesthetics or tapes
  • A special garment will be provided to you for the procedure. You will be advised of the cost of the garment by the staff

Your admission time will approximately be 60 minutes prior to your surgery. You will have fasted for 7 hours prior to this. You will need to be accompanied by a responsible person who would need to be with you for 24 hours after the procedure. You need to bring your garment and loose comfortable clothing – with a top that is either buttoned or zipped at the front – not one that is pulled over the head.

The time it takes to perform the procedure will vary, depending on the quantity of liposuction. After surgery, you will be taken to recovery, where you will be dressed in gauze, foam and your garment. No sutures are required.

You will have some minor discomfort and swelling. The nurse can give you some pain relief for this. The next day your dressings will be changed. You will still be able to shower while the foam that is placed over the dressings is still in place. The foam and dressings are removed at one week post-operatively. You will be required to wear your garment for at least 4 weeks. Rest is strongly advised for at least 2 days, and you will be unable to drive a car for the first week. Swelling and bruising will take around two weeks to subside, and could take even longer. It is important not to expect too much too soon. It will take some time before you can see the final results of your body contour. If you experience any discomfort or extreme pain, you should speak to Dr. Viral Desai. Our caring Mumbai nursing staff will assist you with any needs or concerns that you may have.

Liposuction is performed regularly by Dr. Viral Desai. It is a relatively safe procedure; however, before deciding to go ahead with any surgery it is important to understand that there may be certain risks, complications and side effects. Dr. Viral Desai will discuss these points with youin detail. They include:

  • Wound infection requiring antibiotics
  • Bleeding that may last longer than expected thus requiring a blood transfusion
  • Blood clots can occur
  • Temporary loss of sensation or numbness to the skin- recovers with time
  • Damage to internal organs (extremely rare)
  • Discoloration to the skin
  • Damage to skin can occur delaying the healing process

The possibility of any complication is less than 0.5 %. Liposuction is essentially a superficial surgery and risk free.

You will see a noticeable difference in the shape of your body quite soon after plastic surgery. However, improvement will become even more apparent after about four to six weeks, when most of the swelling has subsided and the final contour will be visible.Approximately 60% percent of swelling is resolved by 1-3 weeks, about 80% by 6 weeks. The skin and fat will contract and re-contour for up to 2-3 months after surgery. No further surgery is recommended to further improve the contour before one year after surgery.

If your expectations are realistic, you will be very pleased with the results of your surgery. You may find that you are more comfortable in a wide variety of clothes and more at ease with your body. And, by eating a healthy diet and getting regular exercise, you can help to maintain your new shape.

Once the fat is removed it will not come back. However, if you put on weight after surgery, the remaining fat cells in the operated as well as the non-operated areas will enlarge and reduce the final result. If you lose weight after liposuction the effect of the liposuction will be further enhanced provided the weight loss is notexcessive.
